Please attach current (minimum 6 months) and previous year's (must be 12 months) Income Statement, Balance Sheet and Statement of Cash Flow information. Be sure to cover all of the information requested in Online Data Input Sheet, such as, Name of the Company requesting your information, SIC code, % of automotive revenue, explanation of extraordinary charges, footnotes on short-term debt, etc.
